CLIENT SERVICE DEPARTMENT

Case Management: Provides basic supportive services to HIV+ individuals and their families around issues of medical care, treatment, housing and nutritional needs. Provides legal/immigration services and weekly support groups. More than 400 HIV infected/affected families are served each year. Case Management has been in existence for nine years.

Treatment Adherence: Seeks to build a collaborative relationship between primary care and mental health services to improve the rates of treatment adherence among the HIV populations and sub-population that are receiving supportive services. This is achieved by engaging and tracking patients through individual, group, and family treatment adherence counseling.

HEALTH, EDUCATION & OUTREACH DEPARTMENT

Our Women’s Empowerment Program is funded by Verizon, Legislators Velmanette Montgomery, Rhoda Jacobs and The Daphne Foundation.  Through this innovative program 15 immigrant women participate in intensive English skills, receive informative monthly workshops on a variety of topics and are linked to Diaspora staff to assist them with any services (food stamps, housing, health insurance, etc.) they may need.

Basic computer skills are also introduced as well as recreational activities to encourage life skills.
 

Nutrition Outreach Education which provides food stamp outreach and assistance in completing applications.
 

Diaspora Healthy Families- Healthy Families New York Program  - Diaspora Community Services began implementing the Healthy Families New York Home Visiting Program in September 2007. This program offers services to expectant families and new parents, beginning prenatally or shortly after the birth of a child. Participants are screened to identify risk factors and stressors that the family may face. Families who participate in the program are offered long-term in-home services until the child is in school or Head Start. Diaspora Healthy Families is a comprehensive prevention program that focuses on the safety of children while at the same time supporting families. The services are easily accessible to isolated, at-risk families and are respectful of cultural and community diversity.

Haitian American Empowerment Program

The Haitian American Empowerment Program effectively answers the varied needs of the Haitian community in Brooklyn by consolidating services in one place, thereby increasing accessibility.  This program addresses the needs of the increasing Haitian population by providing information and referral services (including housing, legal and medical assistance), translation, advocacy, counseling, escorts to appointments, benefits assistance, and educational workshops.   

 

Older Adults Program


The Older Adults Program is established to help people who are 55 and older.  The services include access to health services in and out of the home, support services for older adults, educational, recreational and cultural activities.  The Older Adult Program offers recreational activities including arts and crafts, crochet and fitness.  The program’s main objectives include empowering people 55 and older to be more active in the community, interact with others, and become more aware of the services, entitlements and opportunities that are available to them.

Brooklyn DA's Office (Community Programs)

The Kings County District Attorney's office offers more than 30 community outreach programs for teens, senior citizens, parents, battered women, students, individuals with disabilities and many others. We are committed to crime prevention, intervention and rehabilitative programs for the Brooklyn community. 

Diaspora Community Services is a neighborhood office site and these services are offered on Thursday's between 10:00 a.m. and 3:00 p.m.
