HIV/AIDS Prevention & Support - Wide disparities exist in HIV across New York City communities. This is particularly apparent in Central Brooklyn where incidence is twice that of the Borough and 60% higher than in NYC overall. 

Black men in New York City are 6 times more likely to die of AIDS than white men; black women are 9 times more likely to die of AIDS than white women. Hispanic men and women are 4 times more likely to die of AIDS than white men and women.
 

Diaspora Community Services serves more than 400 HIV infected or affected families each year enabling medical care, treatment and housing.  For 20 years this has been the cornerstone of our service to the Central Brooklyn community.

Case Management: We have provided basic supportive services to HIV+ individuals and their families around issues of medical care, treatment, housing and nutritional needs for over nine years. Case management has also been expanded to include other at-risk populations as well.

Treatment Adherence: Seeks to build a collaborative relationship between primary care and mental health services to improve the rates of treatment adherence among the HIV populations and sub-populations that are receiving supportive services. This is achieved by engaging and tracking patients through individual, group, and family treatment adherence counseling.

  • Medical Benefits Assistance Program (HIV Uninsured Care Programs)**

  • AIDS Drug Assistance Program (ADAP)

  • ADAP Plus (Primary Care)

  • HIV Home Care Program

  • ADAP Plus Insurance Continuation Program (APIC)

**The Uninsured Care Programs are required by law to provide information to our participants about our responsibility to keep information they send to us private and confidential. The "Health Insurance Portability and Accountability Act" (HIPAA) of 1996 requires that we make this notice available. This notice describes the conditions under which we can share information with other people and for what reasons. This notice also tells our participants what their rights are under HIPAA.
